Ischemic stroke in a patient with floating thrombosis of the left ventricular aneurysm and complete atrioventricular block infected with a COVID-19: a case report

Left ventricular (LV) thrombosis is a common complication of heart failure with reduced LV ejection fraction and can be source systemic embolism. The material presents the case 76-year-old woman history myocardial infarction, aneurysm, coronary bypass surgery, who was admitted to hospital ischemic vertebrobasilar stroke. One month prior present hospitalization, patient transient complete atrioventricular block, underwent artery stenting for stable angina pectoris drug-eluting stent, subsequently diagnosed coronavirus disease 2019 (COVID-19), which required further inpatient treatment. Despite dual antithrombotic therapy (antiplatelet+anticoagulant) taken after discharge from hospital, floating thrombus in apical aneurysm. Recurrent conduction disorders (transient block) temporary pacing until elements floated, dual-chamber pacemaker implanted. main feature this formation aneurysm 11 years infarction against background COVID-19 anticoagulant direct oral anticoagulants. At moment, there still no unique opinion on choice patients complicated cardiovascular history. In such cases, regimen should determined by multidisciplinary team, taking into account risks bleeding.
